Description

Amaryl MSR is a fixed-dose oral antidiabetic combination containing glimepiride and sustained-release metformin. It is used with diet and exercise to improve blood glucose control in adults with type 2 diabetes mellitus.

Usage & Safety

For Adult

• Take strictly as prescribed by the physician, usually once daily with or immediately after the main meal.
• Swallow the tablet whole with water; do not crush, chew, or split the sustained-release tablet.
• Dose adjustments should be based on blood glucose response, kidney function, tolerability, and previous antidiabetic therapy.
• Regular monitoring of blood glucose and glycated hemoglobin is recommended.

For Children

• Safety and effectiveness in children and adolescents have not been adequately established.
• Not routinely recommended for patients under 18 years unless specifically prescribed by a specialist.
• No established pediatric weight-based dose or weight limit is available for this fixed-strength combination.

Side Effects

• Common effects may include nausea, diarrhea, abdominal discomfort, loss of appetite, vomiting, and metallic taste.
• Low blood sugar may cause sweating, tremor, hunger, dizziness, weakness, confusion, palpitations, or blurred vision.
• Other possible effects include headache, weight gain, skin rash, and changes in taste.
• Seek urgent medical attention for severe or persistent hypoglycemia, breathing difficulty, extreme weakness, muscle pain, unusual sleepiness, or signs of lactic acidosis.
• Rare but serious reactions include severe allergic skin reactions, liver injury, blood-cell disorders, and vitamin B12 deficiency with long-term metformin use.

Contraindications

• Do not use in patients with type 1 diabetes, diabetic ketoacidosis, diabetic coma, or severe uncontrolled diabetes requiring insulin.
• Contraindicated in severe kidney impairment or acute conditions that may reduce kidney function.
• Do not use in severe liver disease, severe heart or respiratory failure, shock, or conditions associated with tissue hypoxia.
• Avoid in patients with a known allergy to glimepiride, metformin, sulfonylureas, sulfonamides, or any tablet ingredient.
• Temporarily discontinue around certain iodinated contrast-imaging procedures when advised by a healthcare professional.
• Use cautiously in older adults, patients with irregular meals, adrenal or pituitary disorders, or those at increased risk of hypoglycemia.

Precautions & Warnings

• Other antidiabetic medicines and insulin may increase the risk of hypoglycemia.
• Alcohol may increase the risk of hypoglycemia and lactic acidosis and should be avoided or strictly limited.
• Medicines such as beta-blockers, ACE inhibitors, salicylates, some NSAIDs, fluconazole, and other glucose-lowering drugs may enhance the glucose-lowering effect.
• Corticosteroids, thiazide diuretics, sympathomimetics, thyroid medicines, and some hormonal medicines may reduce glucose control.
• Cimetidine, rifampicin, anticoagulants, and other medicines affecting glimepiride metabolism may alter its effect.
• Tell the doctor about all medicines and supplements, especially before contrast imaging or treatment with medicines that can affect kidney function.
• Take with food as directed and avoid skipping meals while using this medicine.

Pregnancy

• Do not use during pregnancy unless specifically prescribed by a specialist; insulin is generally preferred for blood-glucose control during pregnancy.
• Contact the healthcare provider promptly if pregnancy occurs or is planned.
• Breastfeeding is generally not recommended during treatment unless the physician determines that an alternative is unsuitable.
• Infants exposed through breast milk may require monitoring for low blood sugar.
